Looking for a Great Hot Cocoa Mix recipe

I'd like to make some homemade Hot Cocoa Mix gift jars this X-mas. Does anyone have a great recipe or a recommendation for a gourmet-style recipe? I took a quick look around the forum and couldn't find any strings w/ a recipe. Thanks for any help you can give me.

Alton Brown has a similar recipe that uses cocoa instead of grated chocolate in the episode Art of Darkness II: Cocoa

There are also a recipes for homemade chocolate syrup and cocoa brownies that would make an interesting gift basked, put all together.
