It's been a couple years, but I've called up Peoria Packing and they've been able to provide me with a few (back when I was home curing guanciale, this was my source.) So they're worth checking, too.

I've bought a very similar product at one of the small local grocers though it wasn't sliced. In my humble opinion it was quite good and made a decent carbonara and Bucatini all' Amatriciana. It's also considerably cheaper than guanciale although it does have a subtle smoky flavor that sets it apart. I didn't find the smoke overwhelming in the finished dishes but it's there.

Butcher and Larder definitely has it, although you should call early in the week to reserve it. Other places not mentioned yet that would be worth calling are Paulina Market and Chicago Meat Market on Cicero.
